Volunteer Information & Forms

The Cornell School District realizes that parental and guardian involvement is key for any successful school. The District encourages any and all volunteer efforts that benefit our students and the District must ensure we are in compliance with any laws that involve volunteers. Act 153 of 2014 was passed into law that outlines what background clearances are required in order to volunteer and/or work in a school setting. Any and all school district employees are required to have ALL three (3) clearances, and they must be updated every five (5) years.

For any individual who wishes to volunteer in the district, they must also have all three (3) clearances on file in the district, or you must submit two (2) clearances (Pennsylvania Child Abuse History Clearance, Act 151 and Pennsylvania State Police Criminal Record Check, Act 34) and submit a residency affirmation form in lieu of the Act 114 FBI Criminal Background Check. Please see below for the Residency Affidavit. This form must be notarized. The applicant, or volunteer, will bear the responsibility for the procurement and cost for obtaining all clearances.

To qualify for the Residency Affidavit you must have been a Pennsylvania resident for ten (10) years prior to application, and have not been convicted of any crime listed in the Child Protective Services Law.

Please see the following links for information on each clearance.
